As For the mail-in- rebate, (do I need to send 5 separate envelopes with its own receipt) for the 5 appliances I purchased? Or Can I just turn in 1 envelop and 1 receipt for the 5 appliances ?

If all five are on the same receipt, you should be able to fill out one rebate form and send in the receipt and all barcode labels in one envelope. You’re allowed to submit up to five products per household. I’d suggest making a copy of anything you mail in, just for your records.

how do we fill the rebate form out? I plan to buy three, can i use one form for all three or it has to be filled out individually?
Hi Liz! The rebate form states limit 5 per product per household. The instructions for filling out the form and what to include are on the rebate form. I would suggest making copies if you can of the receipt and barcode label for your records before sending in.
