Then entrants must follow the instructions to complete and thereafter submit the entry form by providing the following information: First Name, Last Name, complete Mailing Address (no PO Boxes), Email Address, Phone Number (including Area Code), Date of Birth, Country and the entrants choice of TABASCO gallon jug flavor.
